SurroHub vs. Google Forms, JotForm and Typeform for agency applications

The surrogate application is the front door of an agency. Most agencies build it in Google Forms, JotForm or Typeform. All three make good forms. The question is what happens after someone presses Submit.

Short answer: Google Forms, JotForm and Typeform are excellent general form builders, and JotForm in particular handles long applications, uploads and conditional logic well. But a submission lands as a row or an email, not as a surrogate, egg donor or intended parent with a stage, a checklist and a coordinator. SurroHub's applications are built for surrogacy: you mark answers as red, orange or yellow flags, red-flagged applicants see a kind not-qualified page at the end, every other submission becomes a profile in your pipeline, and the source that brought them is recorded on it.

What each tool does well

JotForm list price as of October 2026, from their pricing page: Bronze $39/month; Gold $129/month, the first plan with HIPAA features. Prices change often; check before you buy. SurroHub is quoted per agency.

Where general form builders strain for an agency

Qualification is the agency's job, not the form's

A surrogate application exists to answer one question kindly and quickly: does she qualify? Form builders can branch on an answer, but each rule is wired by hand, and the reasons someone was flagged rarely travel with her record. Coordinators end up rereading every application to find the BMI, the C-sections and the state she lives in.

Submit is where the work starts

A submission becomes a notification email and a row. Someone still has to create the record, send the welcome message, book the intake call, chase the photos and start the checklist. With Zapier you can automate some of it, and maintain it forever.

Three forms, three tools, no link between them

Surrogates, egg donors and intended parents each need their own form, and an agency often builds them in different tools at different times. Then the egg donor's photos are in one place and the intended parents' preferences in another, with nothing joining them when a match forms.

Ad tracking stops at the thank-you page

Your ads platform learns who submitted a form, not who actually qualified. Agencies spending on Meta ads want the campaign to find more surrogates like the ones who complete and qualify.

When a form builder is still fine

If you take a few applications a month and a coordinator calls every applicant anyway, a well-built JotForm or Google Form is enough. Keep it until retyping submissions and finding the flags becomes a job of its own.

Frequently asked questions

What is the best form builder for a surrogate application?

Among general tools, JotForm handles long applications, uploads and conditional logic best. Agency platforms such as SurroHub go further: the application creates the profile, flags the answers that matter, and starts the follow-up on its own.

Can SurroHub automatically disqualify surrogate applicants?

Yes, using flags you set. Any answer can be marked red, orange or yellow in the form builder. Red-flagged applicants see a not-qualified page at submit; orange and yellow flags appear on the profile for a coordinator to review.

Can I keep my existing application questions?

Yes. Send us your current forms in any format and we set them up in SurroHub, with your questions, wording and branding.

Does SurroHub track which ads bring qualified surrogates?

Yes. Every applicant is tagged with her source, completed applications are sent to Meta as conversion events, and the Google ad click behind each applicant is recorded on her profile.
